Do I need a referral or prescription to start physical therapy?

We can actually care for you without a referral from your doctor! We accept self-pay via cash/check/credit, as well as insurance. However, while most insurance plans allow you to see a physical therapist in the State of Oregon without a doctor’s referral or prescription, your particular insurance may actually require it before you start therapy. (Medicare plans and workers’ compensation insurance do require a doctor’s referral.) So, to make it easier for you, when you call to schedule your appointment, we’ll collect your insurance information and check your benefits for you. We will let you know if a prescription/referral is required so that you can obtain one if necessary. We suggest that you also call your insurance to discover any costs associated with your physical therapy care so that you can budget accordingly without any surprises. We offer payment programs if you happen to have a high deductible, copay or if paying out-of-pocket. We don’t want cost to be a barrier that prevents you from getting the care you need.

What insurance plans do you accept?

We will verify your insurance benefits prior to your first appointment, and educate you about your coverage. Even if we are not a part of your network, we can often match your in-network benefits. Give us a call for details. We are preferred providers with the following insurance companies:
  • Aetna
  • Assurant (Aetna and Cigna Networks only)
  • Blue Cross Blue Shield
  • Cigna
  • Medicare Advantage Plans (Blue Cross, Healthnet, Moda, AARP)
  • Moda Health (Connexus, Synergy, and Beacon Networks only)
  • Multiplan Network
  • Optum Health
  • PacificSource
  • Regence
  • TriCare/TriWest
  • United Health Care
We can also bill auto insurance for motor vehicle accidents, and workers’ compensation for employment injuries. Please stay tuned, as we are currently working on getting contracted with even more networks. We do offer affordable cash-pay rates. What should I bring to physical therapy?

  • Completed forms (See Resource tab)
  • Physical therapy prescription/referral (if you have one)
  • Health insurance card
  • Government-issued identification
  • Imaging study reports: X-rays/MRI/CT scans
  • Appropriate clothing:
    • For lower extremity issues: Shorts are very helpful. We do have shorts at the clinic if you don’t have any or you forget yours at home.
    • For spine/neck issues: Loose fitting sweats/t-shirts. Females: please avoid wearing sports bras.
  • Appropriate equipment (if these items are needed for your care):
    • Assistive devices such as canes/crutches/walkers
    • Shoes/orthotics
    • Braces
Please plan on spending 60-75 minutes for all your physical therapy appointments. This time allows for thorough assessments by your physical therapist and appropriate treatment.
