Do I need a referral or prescription to start physical therapy?

What should I bring to physical therapy?
- Completed forms (See Resource tab)
- Physical therapy prescription/referral (if you have one)
- Health insurance card
- Government-issued identification
- Imaging study reports: X-rays/MRI/CT scans
- Appropriate clothing:
- For lower extremity issues: Shorts are very helpful. We do have shorts at the clinic if you don’t have any or you forget yours at home.
- For spine/neck issues: Loose fitting sweats/t-shirts. Females: please avoid wearing sports bras.
- Appropriate equipment (if these items are needed for your care):
- Assistive devices such as canes/crutches/walkers
- Shoes/orthotics
- Braces

We offer payment programs for those patients that have high deductibles and copays or if paying out-of-pocket. We don’t want cost to be a barrier that prevents you from getting the care you need.
